Step 1. Controlled Ovarian Hyperstimulation (COH)


Naturally, the female body produces one mature egg a month. One egg has a lower chance of becoming fertilized or well developed. Thus, fertility medications are prescribed to control the timing of the egg ripening and to increase the chance of collecting multiple eggs during one of the woman’s menstrual cycles.

The method of COH is to inject a female patient with hormones (gonadotropins) that stimulate the growth of the immature follicle for 7 to 10 days from the second or the third day of her menstruation. A periodical ultrasound will take place to monitor the growth of the follicle. When the follicles are estimated to be fully grown (generally more than 3 follicles that are over 17 mms), the patient will be given another injection (hCG hormone) to trigger the ovulation at night, and 34 to 35 hours after the injection, multiple eggs will be retrieved from the patient.


In the case of a patient who is not responding to this method (if the patient only produces one or two follicles after the ovarian hyperstimulation injection), a single egg which is produced naturally every month will be retrieved for in-vitro fertilization. This natural period method of taking out the egg is inexpensive and can be repeated every month. However, the successful rate of pregnancy is low compared to the method of ovarian hyperstimulation. 


When the female patient is old or has decreased ovarian function, premature follicular rupture often occurs. Therefore, some protocols used to prevent spontaneous ovulation and allows the control of ovulation to the rest exclusively on administration of medications.
 
The long or down regulation protocol
 
From 7 days before the expected menstruation date, a female patient will be injected with hormone (GnRH agonist) that suppresses premature ovulation into her shoulder or lower abdominal area every day. When her menstruation period starts, on the second or third day of the menstruation, the additional other hormone (gonadotrophins) will be injected until the follicles are fully grown and ready to ovulate of egg.
 
1)     Short (flare) protocol
In this protocol, the patient is injected with GnRH agonist (it suppresses premature ovulation) concurrently with gonadotrophins on the second day of the menstruation until she is given a hCG injection.
2)     GnRH antagonist Protocol
This is the latest protocol for superovulation in IVF. Patients start on the gonadotriopins injection from the second or third day of menstruation and when the follicles have grown 12 to 14 mm, then GnRH antagonist is injected in addition, which suppresses the premature ovulation. Both injections will be given until she is ready for the hCG injection.

What is IVF?
IVF (In Vitro Fertilization) is a major treatment for infertility and the most effective procedure of assisted reproductive technology. What is intrauterine insemination (IUI)?
 
Instead of natural intercourse, it is a fertility procedure in which sperm are washed, concentrated, and injected directly into a woman’s uterus. IUI is performed when it is coupled with fertility drugs that recruit multiple follicles (This technique is called controlled ovarian stimulation).
When do we consider artificial insemination?

Mild male infertility (When the motility of the sperm is poor or the sperm count is low)

An issue with cervical mucus may also benefit from artificial insemination since the sperm must pass through the cervix to result in fertilization.
Unexplained infertility
In the case of an impotent male partner
When the anti-sperm antibodies that interfere with sperm motility are found positive
Procedure of Intrauterine Insemination
On your ovulation date, visit our clinic with your partner. Then, fresh semen will be taken from your partner and a semen sample will be washed by the lab to separate the semen from the seminal fluid. This process maximizes the number of sperm with good motility that are placed in the uterus. A catheter is used to inject the processed sperm directly into the uterus. The IUI procedure takes little time and involves minimal discomfort. The woman is generally advised to lie still for a half hour or so after the insemination to prevent seepage.

To diagnose and treat infertility and their causes, different kinds of tests are practiced.
Infertility Workup

Initial assessment
: This testing starts on the first day of visitation to the hospital. An ultrasound test and a Pap smear test are performed to check for any diseases in the reproductive organs.

Semen Analysis (partner’s sperm test)
: Collected after an ejaculatory abstinence period of 2 to 5 days.
: Sperm count, motility, morphology and volume will be measured and examined under the light microscopy.
Hormone Test (blood check)
: A hormone test for checking problems with ovarian function, hypothalamic-pituitary factors, thyroid disorders and adrenal disease.
: 2nd or 3rd day of menstrual period

Hysterosalpingography
: This testing begins as soon as one’s menstruation is finished- Checks for fallopian tubal blockage and abnormality of the uterine cavity (polyp, submucosal fibroids, uterine synechia and so on)
Postcoital test
: This testing is done with a microscope to study the uterine cervical mucus the day after intercourse.
: The test checks the sperm count and their motility. The presence of antibodies in the uterine cervical mucus which may interfere with sperm motility can be estimated from this test.

Follicular monitoring through ultrasound
: Ultrasound can examine the process of follicle development in the ovaries.: It checks for the normal follicle development, and the thickness of the endometrium when ovulating to estimate the exact ovulation date, which can help the fertility rate.
Diagonal Laparoscopy
: This testing begins as soon as one’s menstruation is finished.
: The method is used for unexplained infertility to see the abdominal area by using the endoscope and examining the presence of endometriosis or other conditions.
: This method is also used for treatment purposes when blocked or damaged tubes or endometriosis is suspected.
